Broken Arrow vs Empty Quiver

From what I understand, a Broken Arrow is an accidental event that involves nuclear weapons or nuclear components but does not create the risk of nuclear war. And an Empty Quiver is a nuclear weapon missing from the United States' arsenal. Am I correct?

To quote a phrase from the movie "Broken Arrow",
I don't know what's scarier, losing a nuclear weapon or that it happens so often there's actually a term for it.

Does this mean that in the event that a nuclear weapon would go off 'accidentally', anywhere in the world and humans were casualties...would the Super-Powers who hold nuclear weapons have talks while having their fingers on the button? Would this go the same way for a prequel to a nuclear war? We see it in movies that Presidents of countries will 'chat' before setting the world on fire, but would this be true in a real-life scenario?
